AI agents do not behave like traditional users or services. Consider a DevOps agent asked to investigate a failed deployment. Why the Tool Call Is Where IAM Runs Out of Answers

Execution, not authentication, is the most important part of an agent's workflow. Agent tool calls differ from API requests made by people because they are uncertain, independent, and performed at machine speed. Unlike human users, agent actions lack inherent intent validation, as traditional IAM is technically blind to the purpose behind a tool call. Traditional IAM assumes that once access is granted at login, it remains valid. In agentic settings, that assumption can be harmful.

IAM does not fail entirely at the tool-call layer, but it becomes insufficient without complementary runtime enforcement. Without re-evaluation at execution time, the decision-time gap becomes a real security risk.

Four AI Agent Access Control Failures at the Tool Call

1. Access Is Checked at Session Start, Not at Each Invocation

By relying on session-start validation, traditional IAM enforces a coarse-grained model in which tool calls inherit static permissions regardless of task context. As a result, an agent authorized for "database access" can perform any database operation, regardless of intent. There is no distinction between a safe query and a destructive command.

This gap becomes even more problematic when we consider how those permissions are actually assigned. If access is granted broadly at the beginning of a session, the next question is how those permissions are packaged and delivered to the agent itself. 2. Credentials Are Provisioned for the Agent, Not the Task

Most systems assign credentials at provisioning time (API keys, tokens, service accounts). These credentials don't change from one task to the next, so they can be reused regardless of what the agent is trying to do. They lack contextual awareness, so they cannot adapt to the specific requirements or risks of a given action. Also, they don't expire after a task is done, which means they might be used longer than intended and are more likely to be misused.

In 2026, it is estimated that almost 97% of non-human identities have excessive privileges, and 74% of organizations report that agents receive more access than required. A real-world example shows how risky it is. Replit AI coding agent accidentally erased a production database in 2025 because it had too many permissions. There was no permission for each action; merely general access.

Even if credentials were further limited, there is still a big problem that hasn't been solved: the system still doesn't know why an action is being taken. This brings us to the next failure, where authorization lacks any connection to intent.

3. No Intent Verification Between Request and Execution

IAM validates who is allowed, not why an action is happening. This has become an important failure in AI systems. An agent may misinterpret intent, be manipulated (for example, through prompt injection), or execute logically incorrect actions. Yet from the perspective of IAM, the action still appears valid, and the identity remains authorized. There is no mechanism to verify whether the action aligns with the original user goal or whether the reasoning chain is intact.

If purpose isn't part of the permission process, any approved action carries the same weight, no matter how significant its effect. The real risk is not just incorrect execution, but how far that execution can reach. This shows how important it is to know the blast radius of agent permissions.

4. The Blast Radius Is the Entire Credential Set

When a credential is compromised or misused, the entire permission set is exposed. In agent systems, a single tool call can trigger cascading effects; sub-agents inherit permissions from parent agents, but delegation chains also automatically amplify risk. According to some research, 67% of organizations cannot distinguish AI agent actions from human actions in logs. This creates poor auditability, weak incident response, and a massive blast radius. The agent doesn't need to escalate privileges — it already has them.

Taken together, these failures reveal a consistent pattern: access control decisions are made too early, remain too static, and lack the context needed for safe execution. Addressing any single issue in isolation is not enough. What's required is a shift in where and how enforcement happens, moving from provisioning-time controls to decision-making at the moment each tool is invoked.

Why Standard Fixes Don't Solve AI Agent Access Control

Two main reasons are behind this. First of all, secrets managers secure storage, not execution. They protect credentials at rest and control access to those secrets, but they do not restrict how credentials are used once retrieved or limit permissions for individual actions. After retrieval, credentials are reused across tool calls without further evaluation, leaving runtime risk unaddressed. Second, Role-based access control (RBAC) at the agent level is still provisioning-time control. RBAC defines permissions upfront and reduces overly broad access, but it operates at provisioning time. It does not adapt to task context, real-time intent, or dynamic execution. While RBAC reduces risk, it does not eliminate it.
